From June 2020. No review date but does need updated. Very much a NC document. ?relevance to rest of ICB – link is on the formulary so maybe is relevant to rest of ICB??-only relevant to us as it was what was agreed for NC, on the formulary so that if anyone from NC checks they know what to do. Please update
