Hope all’s well with you.

 

There are 3 changes needed to the PGD web page in the next few days. I thought it might be easier if I sent them all together in one table along with the attachments.

 

 

PGD / letter Change / addition Date to make change / addition
DTaP IPV Hib Hep B (Hexavalent) PGD v5.00 (GOV-16447) Replace GOV 12873 ASAP – expires Weds 24/7
HibMenC PGD v05.00 Notice of Extension of Validity Addition immediately below PGD GOV 12448 ASAP – expires 1st August but not removing existing PGD so no risk of confusion
TdIPV (Revaxis®) PGD v6.00 (GOV 16527) Replace GOV 12925 w/c 29/7 – expires Sun 4th August.

 

 

Can you let me know when you have made the changes?
