We are a team of pharmacy professionals consisting of pharmacists and pharmacy technicians with an aim to optimising medicines use in care homes in the Tees Valley area. We deliver patient-centred medication reviews (shared decision making with patients, family and their GP about which medicines patients should continue to take or stop). We can assist and advise on queries relating to covert medicines administration and swallowing difficulties etc.
Our pharmacy technicians also conduct audits and creates better medicines systems for care homes to reduce waste and inefficiency. We also offer training and support to care home teams to enhance safer administration of medicines.
