Adult Referral Criteria

There are several ways you can be seen by a Registered Dietitian.

Please see the Dietetic Referral Criteria for information on how to refer to the dietitian.

Patients: If you are a patient, please ask a registered healthcare professional or GP to refer you to the dietitian. If you have a question about your appointment, such as changing or cancelling, click here.

Healthcare Professional: Please complete the dietetic referral form and email to cdda-tr.centralbooking@nhs.net. Alternatively complete an electronic referral on Systm One. Please include as much information as possible for the reason for the referral and medical conditions.

We accept referrals for individuals requiring dietetic advice for:

  • Undernutrition - identified as high risk of undernutrition ('MUST' 2 or more) as outlined in the prescribing pathway.
  • Altered consistency food and fluids with evidence of undernutrition ('MUST' of 1 or more)
  • Diabetes (excluding patients on Care +, or Care ++)
  • Inflammatory bowel disease
  • Coeliac disease
  • Irritable bowel syndrome
  • Gastroenterology conditions where nutritional status is compromised (e.g. dumping syndrome, gastroparesis)
  • Confirmed food allergy or intolerance
  • Lipid management
  • Nutritional deficiency
  • Renal (where nutritional status is compromised, unless under a renal MDT)
  • End of life care (please contact 01388 455200 for further guidance)

We currently do not accept referrals for:

  • Weight reduction advice
  • Eating disorder (Please refer to Mental Health Dietetics at Tees Esk and Wear Valley NHS Foundation Trust).

Care Homes: The resident should be high risk ('MUST' 2 or more) and already trialled food based treatments and powdered nutritional supplement. However, URGENTLY refer the resident to the dietitian if recommended thickened fluids and is high risk of undernutrition and requires nutritional supplements. This is to ensure the resident is prescribed the safe consistency of nutritional supplements.

Please refer via the GP or Community Matron/Advanced Nurse Practitioner (ANP) using the Dietetic Referral Form. Please provide the refer with as much relevant information as possible, such as changes in weight, risk of undernutrition/'MUST', types of nutrition treatments already trialled, other medical conditions, such as dysphagia.
