Oxycodone liquid prescribing – important information for EMIS practices

On behalf of the Accountable Officer for Controlled Drugs, we have been asked to inform practices about a number of incidents which have occurred in another Area Team, where prescribers have selected oxycodone 10mg/ml (at the top of pick list on EMIS) instead of oxycodone 5mg/5ml, which has resulted in at least two patients being admitted to hospital.  Please ensure vigilance when prescribing oxycodone liquid, particularly using EMIS.
